To facilitate insurance billing, patients need to complete an insurance form for use by the Center for Allergy and Asthma of Georgia’s billing service. You may complete this form when you are seen in our office or electronically. Our billing office can be reached at: (833) 356-0775

Accepted Insurance at the Center for Allergy and Asthma of Georgia
For Most Insurance Carriers, no referral is needed.
We participate in almost all insurance and managed care programs. In addition to most insurances, we also accept Ambetter, CareSource Marketplace, BCBS Pathway Plans, and VA Optum. Most of these plans require members to pay an office co-payment for each office visit. If your insurance plan requires a co-payment, then it is due at the time of service. We will file your claim to the insurance company for services rendered in our office.
These contracts are frequently reviewed and changed; therefore, we will ask for a copy of your ID card at each visit. Prior to your visit, please contact us or your health plan to verify our participation in your plan.
If there is a problem with verification of your insurance information, full payment for services rendered will be expected at your visit. Therefore, it is incumbent upon you to ensure that you provide the practice with your most recent and correct insurance information. Any secondary insurance cards and information should also be provided to prevent you from being billed for any patient responsibility.
